Crinetics delivered a sharp commercial ramp in Q2 2026, with revenue of $25.118 million—up $14.384 million sequentially—primarily from $24.038 million of PALSONIFY sales, and its $1.14 EPS loss improved from $1.23 in both Q1 2026 and Q2 2025. However, operating loss remained $132.459 million, operating cash use rose to $218.896 million for the first half, and reported near-100% gross margin is temporarily aided by zero-cost inventory. The $1.206 billion liquidity position provides at least 12 months of stated funding runway, but the investment case is now dominated by the pending Vertex merger, expected to close in Q3 2026 and carrying a potential $350.5 million termination fee.

Commercial adoption and reimbursement expanded
PALSONIFY uptake expanded: the company received 245 enrollment forms in Q2, 385 unique HCPs had prescribed the drug within its first three launch quarters, and over 70% of treated patients at quarter-end were on reimbursed therapy.
Sequential loss and EPS improved
The net loss narrowed sequentially by $6.990 million to $120.855 million, while diluted EPS loss of $1.14 improved from the $1.23 loss in Q1 2026 and the $1.23 loss in Q2 2025.
Early gross margin is exceptionally high but temporary
Reported gross margin was approximately 99.4%, using $25.118 million of revenue and $0.154 million of cost of product revenue. Management cautioned that this is temporarily inflated by zero-cost inventory; including it would have added less than $0.3 million of cost in Q2.
Capital position supports near-term operations
Liquidity increased to $1.206 billion in cash, cash equivalents and investment securities at June 30, 2026 from $1.028 billion at December 31, 2025, while working capital rose $201.177 million to $1.164 billion following the January equity offering.
Atumelnant pipeline spending accelerated
Atumelnant investment accelerated: Q2 program R&D was $24.806 million, up $4.539 million sequentially and $13.645 million year over year. Management expects first EQUILIBRIUM-study randomization in the second half of 2026.

Vertex deal may be delayed or fail to close
The new Vertex merger risk factors state that closing remains conditional on shareholder approval, HSR expiration/termination and foreign clearances. Closing is expected in Q3 2026, but either party may terminate if it has not occurred by January 6, 2027, subject to a specified three-month extension.
Merger termination fee and transaction-cost exposure
If the merger agreement ends under specified circumstances, Crinetics may owe Vertex a termination fee of approximately $350.5 million. The filing also states that transaction, legal, financial-advisory and other costs will continue even if the merger does not close.
Large operating losses persist
The business remains deeply loss-making despite revenue growth: Q2 operating loss was $132.459 million and operating margin was approximately negative 527.3%, versus a $140.378 million operating loss in Q1 and $129.112 million in Q2 2025.
Operating cash use increased
Operating cash burn worsened: cash used in operations was $218.896 million in the first six months of 2026, up $44.593 million from $174.303 million in the prior-year period. The company reported an accumulated deficit of $1.666 billion at June 30, 2026.
Reported gross margin is not representative
The reported $0.154 million Q2 product cost does not represent the expected future cost structure because product revenue used zero-cost pre-approval inventory. Management estimates inclusion of that inventory would have increased Q2 cost by less than $0.3 million.
Merger pendency may disrupt operations and retention
The merger agreement restricts certain actions until closing, and the filing warns that counterparties or employees may defer decisions. As of July 22, 2026, the company had 14.3 million outstanding stock options and 2.8 million unvested RSUs, underscoring potential retention and compensation complexity during the transaction.
